Neuartige Muskuloskeletal Therapien

Trotz der hohen Inzidenz von Muskelverletzungen, degenerativen Gelenk- oder Wirbelsäulenerkrankungen und anderen Muskel-Skelett-Erkrankungen wie Sehnen- oder Knochenpathologien ist das grundlegende Thema - die biologische Regeneration - in der klinischen Praxis noch nicht gelöst. Seit 2006 arbeiten wir an translationalen Therapien im Bereich der muskuloskelettalen Regeneration mit dem Ziel, dem Patienten innovative regenerative Ansätze zu bieten.

    Regenerative Plasma- und Zelltherapien in der Orthopädie und Unfallchirurgie


    Advanced regenerative therapies aim to restore acute and degenerative tissue defects. Platelet-rich plasma (PRP) has emerged as a widely applied treatment approach for various forms of osteoarthritis and tendinopathies. To date, robust data on efficacy and product characteristics are however still scarce. While currently still being researched, mesenchymal cell-based therapies may also facilitate tissue regeneration through paracrine immunomodulation. Regenerative medicine in orthopaedics and trauma surgery will have to focus on safety and long-term-efficacy evidence, to advance into clinical practice in the years to come.

    Source and Hub of Inflammation - The Infrapatellar Fat Pad and its Interactions with Articular Tissues during Knee Osteoarthritis


    Knee osteoarthritis, the most prevalent degenerative joint disorder worldwide, is driven by chronic low-grade inflammation and subsequent cartilage degradation. Clinical data on the role of the Hoffa or infrapatellar fat pad in knee osteoarthritis is however scarce. The infrapatellar fat pad is a richly innervated intracapsular, extrasynovial adipose tissue and an abundant source of adipokines and pro-inflammatory and catabolic cytokines, which may contribute to chronic synovial inflammation, cartilage destruction, and subchondral remodeling during knee osteoarthritis. How the infrapatellar fat pad interacts with neighboring tissues is poorly understood. Here, we review available literature with regard to the infrapatellar fat pad's interactions with cartilage, synovium, bone, menisci, ligaments, and nervous tissue during the development and progression of knee osteoarthritis.

    Intramuscular and intratendinous placenta-derived mesenchymal stromal-like cell treatment of a chronic quadriceps tendon rupture


    We report the case of an 80-year-old male patient with a chronic three-time QTR of his right knee. The quadriceps tendon was reconstructed applying a conventional suture anchor repair procedure combined with a synthetic mesh augmentation and additional intramuscular and intratendineous PLX-PAD cell injections as an individualized treatment approach. No adverse events were reported, and excellent radiological and functional outcomes with a passive range of motion of 0/0/120° knee extension-flexion were observed at the 12 month follow-up.

    Proinflammatory and bone protective role of calcitonin gene-related peptide alpha in collagen antibody-induced arthritis


    This study demonstrates a proinflammatory and bone protective role of αCGRP in CAIA. Our data indicate that αCGRP not only enhances joint inflammation, but also controls bone remodelling as part of arthritis resolution. As novel αCGRP inhibitors are currently introduced clinically for the treatment of migraine, their potential impact on RA progression warrants further clinical investigation.

    Immunomodulatory placental-expanded, mesenchymal stromal cells improve muscle function following hip arthroplasty


    In this study we addressed surgical trauma-related muscle injuries resulting from Hip arthroplasty by local intraoperative application of allogeneic placenta-derived, mesenchymal-like adherent cells (PLX-PAD). This cell type was used because of its high regenerative and immunomodulatory potency and we could show that this allogeneic PLX-PAD therapy improved strength and volume of the injured skeletal muscle with a reasonable safety profile. Outcomes could be positively correlated with the modulation of early postoperative stress-related immunological reactions.
